What Is a Care Manager?

   

A Geriatric Care Manager is a professional advocate that addresses issues related to aging and elder care. Geriatric Care Managers work with older adults and their familes to create a joint care plan the meets their specific needs.

Care Managers assist adult children of aging parents, others providing elder care, or the individuals themselves.

 

When Is a Care Manager Called For?

   

Some typical situations in which a Care Manager is indicated:

  • An elderly family members is unsafe at home.
  • Basic daily needs are not being met, such as grooming, cooking, or transportation.
  • Health care needs are consuming the lives of the caregivers.
  • There is difficulty in managing bills and finances.

A Care Manager will meet with those involved to determine if care management services would be helpful.

 

What Services Are Provided?

   

The Care Management relationship can range from occasional recommendations of service providers to ongoing coordination of life management needs. Here are some types of services we can provide:

  • Development of a care plan
  • Referrals and recommendations for quality service providers
  • Assessments to determine capabilities and needs
  • Advocacy and representation with health care agencies and service providers
  • Coordination of services and service providers
  • Education and other direct support services

Melissa Cronin, MS, LCPC, CRC
Senior Care Manager

Melissa is a Licensed Clinical Professional Counselor in the State of Illinois and is a Nationally Certified Rehabilitation Counselor with specialized training in Geriatric Care. She is a graduate of the University of Illinois with a Master's Degree in Rehabilitation Counseling and a Bachelor's Degree in Social Work.

     
 

Amy Wingo, MS, CRC
Senior Care Manager

Amy is a graduate of the University of Illinois with a Master's Degree in Rehabilitation and a Bachelor's Degree in Psychology.

Both have extensive experience in service coordination and counseling for individuals across the lifespan and specialize in issues related to disability and vocation.

     
 

William Gingold, PhD, MHA, CHE, CSA, QMHP, LP
Senior Advisor

Dr. Gingold is a psychologist and gerontologist. He is also Clinical Assistant Professor and Director of Family Medicine Research at the University of Illinois.
