Passport to Wellness™ is a comprehensive community-based screening assessment and education program for mature adults (55+).

The program is flexible and adaptable to your organization's needs and objectives. You can design a customized Passport to Wellness for your target group by selecting appropriate choices from an extensive menu of screening assessment tools and protocols.

CHALLENGES AND CHOICES™

The Challenges and Choices: A Problem Identification Tool is a two-part discovery and awareness vehicle designed to build and support the Caregivers and the elder care community. By shedding light on challenging behaviors, issues, and concerns associated with care for care recipients, Challenges and Choices serves as a practical guide for thoughtful responses and objectively pinpointing key issues, concerns, and problems that the clinician or other professional service providers can use to accommodate and/or help ameliorate.

CAREGIVER SUPPORTS

A number of tools have been developed to reach out to and assess the caregiving community. One example is an comprehensive family caregiver survey.

LOOK AND LAUGH KIT

Laughter is the best medicine is more than just quaint saying. It has research behind it. The Look and Laugh Kit has been developed specifically for older adults to combat social isolation and increase overall wellness.

LONG TERM CARE ADMINISTRATION SURVEY

A comprehensive survey was conducted across multiple service providers to gauge the needs of administrative staff to further develop training and resources that ensure personal success and quality of care.

SAGE FOR AGING

The Sage for Aging database contains practical advice for assisting with common tasks and recurring behavioral issues in caring for the elderly. This site is currently being upgraded and expected to be available again in 2024.
